Therapist applicants must meet all the following requirements:
* Hold active, independent clinical licensure (LCSW, LMFT, LPC, etc)
* Own individual professional malpractice insurance.
* Have individual NPI & CAQH numbers (& complete a CAQH application).
* Pass background checks.
To expedite the application process, please have your clinical licensure number(s), proof of malpractice insurance, NPI & CAQH number(s), a government-issued ID, and a resume ready.
